Full-Time Permanent EDUCATION/
Experience:
Masters degree in social work from an accredited school of social work.
Current clinical level social work license in the State of New Mexico (LCSW or PsyD).
Basic computer skills.

years related experience in crisis intervention and/or medical social work.
Essential Functions:
Provides evidenced based behavioral health consultation and interventions.
Is available for same day and scheduled initial consultations with referred patients.
Provides brief/focused counseling and support for patients and families through evidenced based interventions and treatment modalities.
Performs brief/focused follow-up visits and phone calls for selected patients.
Adds to the multidisciplinary care plan with patient and family input.
Provides psycho-education for patients during individual or group visits.
Triages patients with severe or high-risk behavioral health problems and refers such patients to community resources and/or specialty behavioral health services.
Conducts risk assessments as indicated.
As needed, provides resources to meet financial, transportation, housing and other needs.
Assists patient in setting up advanced directives.
Assists in the development of clinical pathways, group classes, and behavior focused practice protocols.
Maintains a visible presence in the clinic and attends clinic meetings.
Documents consultative notes in the electronic medical record:
outlining assessment findings, interventions delivered, and recommendations made to the patient and primary care provider; in a timely manner to comply with all administrative regulations.
Maintains strict confidentiality guidelines regarding all patients, conversations and referrals in line with local, state and federal statutes, and organizational policies.
